MNM Fatality – 10/7/22

On October 7, 2022, a mechanic was fatally injured when he was engulfed in material after he entered an air separator to clear a blockage. 

Best Practices: 

  • Install mechanical flow-enhancing devices, such as mechanical vibratory devices, that can maintain material flow and prevent the need for miners to enter a confined space.
  • Mine operators should establish procedures to clear blockages and train all miners in the procedures.
  • Make sure miners wear a safety belt or harness equipped with a lifeline when entering a confined space, and that a second miner, similarly equipped, is attending the lifeline.

This is the 24th fatality reported in 2022, and the first classified as “Confined Space.”

MNM Fatality – 10/1/22

On October 1, 2022, a miner died while using a crane to remove a haul truck engine.  The auxiliary line broke, causing the hook and ball assembly to fall and strike the miner.

Best Practices: 

  • Make sure cranes have functional anti-two blocking devices to automatically shut off the crane when the rigging on the hoist line gets close to the sheave at the end of the crane boom.
  • Make sure miners stay clear of suspended loads and use taglines when necessary for steadying or guiding suspended loads.
  • Make sure miners conduct thorough pre-operational inspections of all machinery, equipment, and tools prior to use.

MNM Fatality – 9/28/22

On September 28, 2022, a miner died when he was engulfed in a collapsed stockpile.  The miner was working close to the toe of the stockpile to remove material off the top of a surge tunnel’s feeder to clear a blockage.

Best Practices: 

  • Operators should have procedures to safely clear blockages from feeders, and train miners to stay out of areas where there is a danger of falling or sliding material.
    • Equip feeders with mechanical clearing devices to prevent exposing miners to hazards of falling or sliding material.
  • Make sure miners trim stockpiles to prevent hazards.

MNM Fatality 8/4/22

On August 4, 2022, a mine manager died while performing maintenance on a bulldozer.  While kneeling on the bulldozer’s track, the victim accidentally engaged the lever that put the bulldozer in reverse.  The bulldozer track moved the victim to the rear of the bulldozer where he was run over.

Best Practices: 

  • Operators should train miners on safe maintenance procedures:
    • turn off the engine
    • block equipment against motion
  • Operators should make sure miners are in a safe location in relation to machine parts that can move.

MNM Fatality – 9/9/22

On September 9, 2022, a dredge operator drowned while working in a dredge pond.

Best Practices: 

  • Make sure a competent person conducts workplace examinations before miners begin work to identify conditions that may adversely affect safety.
  • Make sure miners wear life jackets or belts where there is danger of falling into water.
  • Make sure miners maintain communication when working alone.

MNM Fatality – 7/21/22

On July 21, 2022, a miner received fatal injuries when his right arm became entangled in an auger (screw) conveyor. 

Best Practices: 

  • Secure all conveyor covers in place during normal operation.  Keep tools, clothing, and body parts away from moving conveyors.
  • De-energize, lock out, tag out, and block machinery against hazardous motion before performing repairs or maintenance.  Never perform work on a moving conveyor.
  • Examine work areas and equipment.  Report defects to miners and assure defects are corrected and recorded.  Test emergency shut-off devices frequently.

MNM Fatality – 6/20/22 Surface

On June 20, 2022, a contract driller was working outside of his drill when he fell from the top of a highwall.

Best Practices: 

  • Wear fall protection when there is a danger of falling.  Assure fall protection has a suitable fall arrest and a secure anchorage system.
  • Train miners to properly use their personal protective equipment and to recognize potential hazards from falls and to safely perform tasks.
  • Provide communication systems when assigning miners to work alone.

MNM Fatality – 6/20/22 Underground

A 50 year-old miner died when the excavator he was operating underground slid over an elevated loading pad and was engulfed by lime dust.

Best Practices: 

•    Conduct workplace examinations prior to beginning work and assure hazards are corrected.
•    Train miners to identify and report hazards and stay clear of potentially unstable areas.

MNM Fatality – 6/17/22

On June 17, 2022, a contract miner died when the compactor he was operating overturned, pinning him beneath the cab.  As the miner was backing up, the left tire went off the edge of a four-foot embankment, causing the compactor to overturn.

Best Practices: 

  • Wear seat belts at all times when operating equipment.
  • Train miners to perform tasks safely, and to recognize potential hazards.

MNM Fatality – 1/28/22

On January 28, 2022, while driving downhill, the 56 year-old driver of a concrete mixer truck was fatally injured after he lost control of the truck.  The truck overturned and the driver was ejected from the truck.  Another miner, who was in the truck, was also ejected and suffered serious injuries.

Best Practices: 

  • Each shift, inspect mobile equipment before placing it into operation.  Correct defects that pose a hazard to miners.  Tag out mobile equipment that can’t be corrected.
  • Assure brakes can stop and hold mobile equipment with its typical load on the maximum grade it travels.
  • Always wear seat belts when operating mobile equipment.
  • Maintain control of mobile equipment and drive at safe speeds.
