On January 30, 2025, a miner died when a front-end loader operator unknowingly lowered a pallet of stone onto him.

Best Practices

  • Develop traffic rules for mobile equipment and miners on foot. Ensure the Safety Program for Surface Mobile Equipment includes actions taken to identify hazards and risks to reduce fatalities.
  • Install collision warning technologies and added safety features such as cameras, sensors and radar.
  • Ensure adequate clearance and visibility when operating mobile equipment and be aware of where all persons on foot are located.
  • Wear high visibility clothing and communicate your location and intended movements to mobile equipment operators.  Ensure they acknowledge your presence before you travel near mobile equipment.
  • Train all persons to recognize the limited visibility and blind areas inherent to the operation of self-propelled mobile equipment.

Additional Information

This is the fifth fatality reported in 2025, and the second classified as “Powered Haulage.”

MNM Fatality – 9/6/24

On September 6, 2024, a miner died when the haul truck he was operating traveled off the mine road and crashed into a residence.

Best Practices

  • Perform pre-operational examinations of mobile equipment.
  • Ensure berms and guardrails are at least as high as the mid-axle height of the largest mobile equipment using the roadway.  Use larger berms and runaway safety features where mobile equipment can likely lose control.
  • Maintain control of self-propelled mobile equipment while it is in motion.
  • Always wear a seatbelt and remain in the cab with your seatbelt on while operating mobile equipment.  Never attempt to exit or jump from an out-of-control vehicle.
  • Ensure miners are task trained in the safe operation of all equipment they use.

Additional Information

This is the 20th fatality reported in 2024, and the ninth classified as “Powered Haulage.”

MNM Fatality – 8/22/24 PA

On August 22, 2024, a contractor was found deceased in a shallow pool of water at the base of a highwall.

Best Practices

  • Conduct workplace examinations to identify conditions that may adversely affect the safety or health of miners.
  • Provide and maintain safe access to all working places. 
  • Ensure that miners have communication systems available when assigned to work alone.
  • Train miners on safe work procedures and to identify hazards associated with their assigned tasks.

This is the 16th fatality reported in 2024, and the first classified as “Drowning.”

MNM Fatality – 11/17/23

On November 17, 2023, a miner died when the leg of a rock saw stand that he was assisting to unload, shifted and struck him.

Best Practices

  • Review safe procedures with all miners involved prior to conducting a non-routine task.
  • Use proper rigging and lifting procedures when moving machine parts.
  • Load and unload materials on smooth, level surfaces to ensure the material does not shift or fall over.
  • Ensure miners are positioned in a safe location and away from potential hazards when moving or positioning equipment.

MNM Fatality – 8/5/23

On August 5, 2023, a piece of granite fell, striking two miners, killing one and seriously injuring the other.

Best Practices

  • Examine work areas to identify loose ground or unstable conditions before work begins and as conditions change.  Report hazards and do not work in unsafe conditions.
  • Correct unsafe conditions or barricade areas to prevent access before beginning work.
  • Consider mining methods that do not require miners to work or travel near the base of a highwall.

MNM Fatality – 4/9/23

On April 9, 2023, a miner died while mounting off road truck tires on rims.  The miner was in the process of mounting a tire on its rim when another tire’s outer lock ring dislodged, became airborne and struck the victim in the head.

Best Practices

  • Use a tire safety cage (or other tire restraint device) when inflating a truck tire that is not on the truck.  Stand away from the inflating tire and out of the wheel and air blast trajectory during the inflation process.
  • Check tires and rims for damage and defects before performing work.
  • Before repairing tires, completely deflate the tires by removing the valve core and checking the valve stem to make sure it’s clear. 
  • Follow the manufacturer’s procedures and make sure tire and rim components are compatible.
  • Train miners who service tire and rim components.

MNM Fatality – 7/28/21

On July 28, 2021, a 42 year old ledge foreman with 12 years 7 weeks experience* was standing on a rock ledge at a mine with 8 employees in Elberton, GA* to extract dimensional stone when a triangular section of the rock broke off, causing the miner to fall approximately 35 feet.

Best Practices: 

  • Use fall protection when a potential fall hazard exists.  Ensure fall protection has a suitable fall arrest and secure anchorage system.
  • Examine working places to identify loose ground or unstable conditions before work begins, after blasting, and as changing ground conditions warrant.  Ensure examiners have adequate training and experience to recognize potential hazards.
  • Assess risks and control hazards before beginning work activities.  Remain a safe distance from cracks and any sign of unstable ground conditions.
  • Assure a safe means of access is provided and maintained to all working places.  Use personnel lifts and ladders, as required.
  • Train miners and ensure they perform work safely, use tools properly, and utilize personal protective equipment correctly.

On May 13, 2019, a 59-year-old supervisor with 40 years of experience was fatally injured when the stationary crane he was operating fell 85 feet into the quarry.

Best Practices: 

  • Ensure all safety devices are functional.
  • Conduct a visual inspection of the equipment, load, and rigging prior to placing equipment in operation..
  • Conduct a visual inspection of site conditions and potential hazards.

Fatality #12 for Metal/Nonmetal Mining 2018

On October 19, 2018, a 63-year old quarry manager, with 17 years of experience, was fatally injured when he lost control of the haul truck he was driving.  The victim was operating a haul truck down a steep grade and traveled through a berm and over a short drop-off.  The victim was not wearing a seat belt.

Best Practices

  • Always wear seat belts when operating mobile equipment.
  • Maintain control and stay alert when operating mobile equipment.
  • Conduct adequate pre-operational checks and correct any defects affecting safety in a timely manner prior to operating mobile equipment.
  • Operate mobile equipment at speeds consistent with the conditions of roadways, tracks, grades, clearance, visibility, curves, and traffic.
  • Ensure that berms are adequate for the vehicles present on site.  Among other things, they should be constructed of appropriate materials, be of adequate height, and be built on firm ground.

Fatality #11 for Metal/Nonmetal Mining 2018

On October 11, 2018, a 26-year old miner with 48 weeks of experience at the mine was fatally injured as a result of falling from on top of a previously cut block of granite.  The victim was in the process of separating the cut block of granite from the highwall when the cut block suddenly slid out.  The movement caused the miner, who was not wearing fall protection, to lose his balance and fall between the rock and the highwall causing fatal injuries.

Best Practices

  • Install fall protection systems that allow safe movement to perform work.
  • Always conduct examinations of working places in order to identify loose ground or unstable conditions before work begins and as changing ground conditions warrant.
  • Ensure that the person conducting the examination has the training and experience to recognize potential hazards.
  • Discuss work procedures and identify all hazards associated with working near highwalls along with the methods to protect personnel.
  • Do not place yourself in a position that will expose you to hazards while performing work tasks.
