Coal Fatality – 8/17/22

MINE FATALITY – On August 17, 2022, a general inside laborer died when he was caught between a supply car and its coupler.  The victim was sitting on the supply car which was coupled to a locomotive parked in a track spur.  The locomotive was struck by another locomotive pulling three loaded cars into the mine.  The impact knocked the victim off the supply car, killing him.

Best Practices: 

  • Assure track switches are in the proper position for your direction of travel and the latches are in contact with the rail.
  • Make sure miners are in a safe location away from equipment parked in a track spur when other equipment is passing along the main rail line.
  • Make sure miners communicate their location and intended movements with the dispatcher.  Repeat the switch alignment back to the dispatcher, where applicable.

This is the 18th fatality reported in 2022, and the fourth classified as “Powered Haulage.”

MNM Fatality – 1/28/22

On January 28, 2022, while driving downhill, the 56 year-old driver of a concrete mixer truck was fatally injured after he lost control of the truck.  The truck overturned and the driver was ejected from the truck.  Another miner, who was in the truck, was also ejected and suffered serious injuries.

Best Practices: 

  • Each shift, inspect mobile equipment before placing it into operation.  Correct defects that pose a hazard to miners.  Tag out mobile equipment that can’t be corrected.
  • Assure brakes can stop and hold mobile equipment with its typical load on the maximum grade it travels.
  • Always wear seat belts when operating mobile equipment.
  • Maintain control of mobile equipment and drive at safe speeds.

Additional Information: 

This is the sixth fatality reported in 2022, and the second classified as “Powered Haulage.”

MNM Fatality – 2/14/22

On February 14, 2022, a 34 year-old maintenance technician died while driving a lube truck underground.  The truck over traveled the edge of a stope and fell approximately 60 feet into the stope drift.

Best Practices: 

  • Provide berms, bumper blocks, safety hooks, or similar impeding devices at dumping locations where there is a hazard of over travel.
  • Examine working places before work begins for conditions that may adversely affect safety and health

Additional Information: 

This is the seventh fatality reported in 2022, and the third classified as “Powered Haulage.”

MNM Fatality – 1/26/22

On January 26, 2022, a dump truck rolled backward onto a 54 year-old miner while the miner was attempting to troubleshoot a brake issue.  The dump truck operator was unaware that the miner was under the truck when he released the parking brake, allowing the dump truck to roll backward. 

Best Practices: 

  • Block machinery or equipment against hazardous motion before performing repairs or maintenance.
  • Communicate with mobile equipment operators and assure they acknowledge your presence, before traveling near mobile equipment.  Do not assume that mobile equipment operators know that you are in the vicinity.

MNM Fatality – 12/13/21 Surface

On December 13, 2021, a customer truck driver (victim) stopped at a designated tarping area and was assisting another driver with a rear trailer indicator light.  The victim’s truck moved forward and pinned him against the back of the other driver’s truck.

Best Practices: 

  • Do not leave mobile equipment unattended unless the transmission is in the park position, the parking brake is set, and the trailer brakes are engaged.
  • Block, or secure from movement, mobile equipment parked on a grade.
  • Never position yourself in hazardous areas around mobile equipment parked on a grade that is not blocked or secured from movement.
  • Train customer truck drivers on site-specific hazards.

MNM Fatality – 12/3/21

On December 3, 2021, a miner was fatally injured when he became entangled in the return idler on the belt conveyor under a portable crusher plant. 

Best Practices: 

  • Before performing maintenance and repair work near belt conveyors:
    • Remove power from the belt drive.
    • Securely block equipment against hazardous motion in accordance with manufacturer’s instructions.
  • Guard moving machine parts to protect miners from contacting moving parts.
  • Provide and maintain a safe means of access to all working places.
  • Conduct thorough examinations of equipment. Report defects and do not work in unsafe conditions.
  • Train miners to assess risks and control hazards before beginning work on belt conveyors.

Coal Fatality – 11/1/21

On November 1, 2021, an electrician with 25 years of mining experience was fatally injured at a mine with 334 employees* while traveling down a mine slope.  He lost control of a four-passenger rubber-tired personnel carrier, and the vehicle crashed at the bottom of the slope, pinning the victim underneath.

Best Practices: 

  • Immediately remove equipment from service when defects affecting safety are found.
  • Conduct adequate preoperational checks and weekly examinations of mobile electrical equipment.  Correct any defects affecting safety before operating mobile equipment.
  • Maintain control and stay alert when operating mobile equipment.
  • Maintain roadways free of excessive water, mud, and other conditions that impact an equipment operator’s ability to control mobile equipment.
  • Operate mobile equipment at speeds consistent with the conditions of roadways, grades, clearance, and visibility.
  • Never rely on Regenerative Braking as a substitute for keeping brakes properly maintained.

MNM Fatality – 9/21/21

On September 21, 2021, a 68-year-old contract truck driver with 20 years of experience was fatally injured while operating a haul truck.  The victim was found lying in front of his truck near the edge of a haul road.  The truck was upright and in the opposite direction of the expected route of travel.

Best Practices: 

  • Establish a site traffic plan to include traffic routes, speed limits, and access points.  Train miners to follow all traffic controls.
  • Conduct pre-operational examinations to identify and repair defects that may affect the safe operation of equipment before placing equipment into service.
  • Operate mobile equipment at speeds consistent with conditions of roadways, grades, curves, and traffic.
  • Maintain control while operating mobile equipment.  Never exceed a vehicle’s design capabilities, operating ranges, load limits, and safety features.
  • Always wear a seat belt when operating mobile equipment.
  • Never exit a moving vehicle.  Remain in the seat with your seat belt secured.

Proposed Mobile Powered Haulage Rule

DOL – MSHA announced a proposed rule to require mine operators employing six or more miners to develop a written safety program for mobile and powered haulage equipment (excluding belt conveyors) at surface mines and surface areas of underground mines. This proposed rule is one of several actions MSHA has taken to reduce fatal and nonfatal injuries involving surface mobile equipment used at mines and to improve safety and health.

Under the proposed rule, mine operators would implement a written safety program including actions to identify hazards and risks to reduce accidents, injuries and fatalities related to surface mobile equipment. Mine operators would have the flexibility to devise a safety program for their specific mining conditions and operations. In addition, MSHA would encourage its state grantees to provide training to address hazards and risks involving surface mobile equipment in small mining operations.

Comments must be submitted by Nov. 8, 2021.

Coal Fatality – 8/11/21

On August 11, 2021, a 53-year-old contract truck driver with ten years’ experience was fatally injured while conducting a pre-operational examination of a truck at a mine in Mill Creek WV with 28 employees and 20 contractors*.  The rear wheels of the vehicle struck the truck driver when the truck rolled forward.

Best Practices: 

  • Block mobile equipment against motion.  Adequately chock wheels or turn wheels into a bank.
  • Use specially designed truck-wheel chocks of the appropriate size and material to hold the vehicle securely.  Do not use lumber, cinder blocks, rocks, or other makeshift items to chock.
  • Never position yourself in hazardous areas around equipment parked on a grade that is not blocked or secured from movement.
  • Maintain the equipment’s braking systems.  Perform repairs and adjustments when necessary and follow the manufacturer’s recommendations.  Do not exceed the manufacturer’s load limits.
