MNM Fatality – 1/30/25 sand

On January 30, 2025, a miner died when he became entangled in a log washer.  The victim was starting the log washer when he lost his balance and fell into the log washer. 

Best Practices

  • Ensure equipment and safety devices are maintained in safe working condition.
  • Ensure miners are clear and free from hazards when starting equipment and performing tasks.
  • Always use fall protection equipment, safety belts and lines or personnel lifts when working at heights and near openings where there is a danger of falling.
  • Lock Out and Tag Out machinery against hazardous motion.
  • Maintain equipment in accordance with manufacturer’s means to care for and service equipment.
  • Train miners in the recognition of hazards and safe work procedures.

MNM Fatality – 1/3/25

On January 3, 2025, a miner died when an excavated trench collapsed and engulfed him.  The victim was replacing a 12-inch discharge line in the trench when the wall collapsed.

Best Practices

  • Stay clear of potentially unstable areas.  Do not enter trenches if the trench walls are not properly supported for the full height or sloped to a safe angle.
  • Establish and discuss safe work procedures before beginning work.  Identify and control all hazards associated with the work to be performed and the methods to properly protect persons.
  • Follow OSHA Trenching and Excavation Safety Guidelines.

Additional Information

This is the first fatality reported in 2025, and the first classified as “Falling, Rolling, or Sliding Rock / Material of Any Kind.”

MNM Fatality – 11/16/24

On November 16, 2024, a miner died when he became entangled in a log washer.   The miner was last seen standing on a deck and using a water hose to wash material out of the log washer to clear a blockage. 

Best Practices

  • De-energize machinery and block moving machine parts from hazardous motion before conducting maintenance or repairs. 
  • Ensure miners are in a safe location before moving equipment and components.
  • Identify hazards associated with the task, review those hazards with all miners involved, and implement measures to protect miners.
  • Train all miners in the recognition of hazards and safe performance of tasks.

MNM Fatality 4/15/24

On April 15, 2024, a miner died when the tail section of a collapsible belt conveyor fell and pinned him between the tail section and middle section.  The miner was working as part of a crew to tear down the conveyor in preparation to be moved.

Best Practices

  • Block all equipment against hazardous motion before beginning work.
  • Stay clear of suspended loads.
  • Never enter or work in pinch points or Red Zones around equipment.
  • Identify hazards associated with the task, review those hazards with all miners involved, and implement measures to protect miners.
  • Train all miners in the safe performance of tasks.

MNM Fatality – 12/14/23 UT

On December 14, 2023, a contractor died while delivering parts to the mine when an all-terrain telehandler pulling cable, tipped over, striking him.

Best Practices

  • Do not exceed the load radius and load limits of lifting equipment.
  • Ensure miners position themselves in a safe manner while working around equipment.
  • Barricade and sign affected areas in case equipment or loads fail or tip.
  • Ensure visitors entering the mine receive site specific hazard awareness training.
  • Maintain good communication between co-workers.

MNM Fatality – 8/21/23

On August 21, 2023 a crusher lid that was being moved into place struck a miner when the rigging broke. The miner died from his injuries on August 23, 2023.

Best Practices

  • Do not work under suspended loads.
  • Use properly rated lifting equipment and ensure that the load is well secured.
  • Attach tag lines to suspended loads to steady or guide the load.
  • Communicate lift plans to all persons working in the lift zone. Follow manufacturer’s recommended work procedures.

MNM Fatality – 8/24/23

On August 24, 2023, a miner died while preparing a wash plant for relocation. In the process of lowering the feed box into the travel position, the miner was pinned between the feed box and the handrail.

Best Practices

  • Block machinery components against motion before beginning maintenance or repairs.
  • Do not work under suspended loads.
  • When conducting a non-routine task, review safe procedures before starting work and ensure all safety components are in place and working.

MNM Fatality – 8/5/23

On August 5, 2023, a customer truck driver fell from a large container mounted on the trailer of his truck. After opening the container lid, the driver slipped while descending the container.  The driver died from his injuries on August 7, 2023.

Best Practices

  • Provide fall protection or safe means of access to miners, contractors, and customers who need to gain access to containers or tankers if there is danger of falling.
  • Ensure miners, contractors, and customers wear proper slip resistant footwear and use three points of contact when accessing elevated surfaces.
  • Provide site-specific hazard awareness training to customers, including commercial over-the-road truck drivers.

MNM Fatality – 6/22/23

On June 22, 2023, a contract bulldozer operator died when the bulldozer he was operating backed off the edge of a highwall.  The bulldozer operator was clearing the bench in preparation for drilling and blasting and had removed the berms along the edge.

Best Practices

  • Install berms, signs, or devices to identify the edge of benches.
  • Perform workplace examinations and correct hazardous conditions before beginning work and as ground conditions warrant. 
  • Train equipment operators to identify dangerous conditions and to keep the bulldozer blade between them and the edge near drop-offs.

MNM Fatality – 6/1/23

On June 1, 2023, a customer truck driver died when he was struck by a component of his truck’s air bag height/ride suspension system while performing maintenance. 

Best Practices

  • Follow manufacturer’s recommended procedures when performing maintenance on suspension system components.
  • Release all stored energy from components before beginning maintenance.
  • Position yourself in a safe location while pressurizing systems.
  • Train miners in safe work procedures and hazard recognition.  Monitor personnel routinely to ensure they follow safe work procedures.
